At Worcester State, the accommodation process is student-initiated. Please use this form to initiate your request for academic, housing, and/or dietary accommodations.

If you are requesting a private sleeping area (e.g., a single), please include why typical roommate adjustments (such as seeking out quiet study spaces on-campus, using headphones, wearing a sleeping mask, or negotiating shared use of space) would not resolve the access concerns. If you are requesting an assistance animal, please list the types of work, tasks, assistance and/or therapeutic emotional support the animal provides in relation to your disability.
Housing accommodations are based on several factors, including each student's submitted materials, the number of students seeking housing accommodations, the needs of the current population living on campus, and available housing stock at the time of approval.
While many dietary needs can be accommodated by Campus Dining. You can contact the Chartwells team directly to discuss available food options and their capacity to meet specific dietary needs.
SAS also works with students who are requesting a meal plan modification (exemption or reduced meal plan) as a disability-related accommodation. Please note that meal plan accommodations, including a modification or exemption from mandatory participation in a meal plan are rare. Exemptions are made solely for documented medical conditions, including food allergies, that require restricted, medically necessitated diets that cannot be accommodated by Chartwells.
